The use of technology to monitor the patient’s condition at home was integrated into the existing care for patients with COPD.
This aimed to:

- Increase clinical resource capacity available within specialist and community teams to provide COPD services.
- Introduce monitoring of essential vital signs for patient with COPD.
- Improve self-management by engaging, educating and empowering patients and carers in understanding and managing their own COPD.
