The use of technology to monitor the patient’s condition at home was integrated into the existing care for patients with COPD.

This aimed to:

  • Prevent and reduce unplanned crisis episodes for people with COPD requiring hospital admission or re-admission.
  • Increase clinical resource capacity available within specialist and community teams to provide COPD services.
  • Introduce monitoring of essential vital signs for patient with COPD.
  • Improve self-management by engaging, educating and empowering patients and carers in understanding and managing their own COPD.
