Each of the pilot sites:

  • Through redesign, established telehealth pathways in their local areas.
  • Developed local stakeholder engagement in preparation for deployment and roll out.
  • Procured different types of monitoring technology – including SMS
  • Integrated the use of telehealth across existing mainstream clinical services.
  • Prepared detailed plans to support service delivery, workforce implications and workflow changes.
  • Delivered Telehealth Services to patients through U4H for a minimum of 10 months.

Integrating telehealth offered alternative care options and different levels of monitoring to staff, patients, family and carers. Partners provided management and stakeholder engagement – contributing to the evaluation of the project.
