A national community of practice (HMHM Action Group) has been established to promote knowledge exchange and adoption of HMHM across Scotland.

 

The key objectives of the Community of Practice are:

  • To actively promote and share learning from the National Technology Enabled Care Programme in Scotland
  • Provide a forum for Knowledge Transfer and peer support network for partners
  • Supporting local and national implementation of HMHM and benefit realisation
  • Share evidence and good practice examples
  • Consensus building and support for developmental of national work supporting scaling up of Home & Mobile Health Monitoring.

Throughout the year, HMHM Knowledge Exchange events are held to support this programme of work.

Locations will vary dependent on availability at the time. The action group meetings will be held in Edinburgh or Glasgow. Where possible, we also try to ensure VC connections are available for these meetings.
